What to Expect as a Club Member

Club Members can expect to be welcomed into a friendly, safe and relaxed atmosphere. Register for 1 day up to 6 days a week (pending suitability/availability), with flexible arrival and departure times to accommodate each family’s schedule.

Activities help Club Members socialize, engage in the world around them, and try new things.

Club Members may participate in any of the following activities:

  • 3D immersion room meditations, exercise classes, and more (at the Soper Club)
  • Music sessions with singing, instrument playing and/or music therapy
  • Sports games and exercise classes
  • Baking
  • Discussion groups and word games
  • Art projects such as painting, crafts
  • Gardening
  • Pet therapy
  • Drama-based skits and performances
  • Special events and themed days
  • Guest speakers and entertainers
  • Relaxation or quiet time
  • Coffee and conversation
  • Applied social research projects through the Center for Elder Research at Sheridan

Club Members will also be supported in the following ways:

  • Assistance with eating, walking, toileting and taking medications (no injections)
  • Nutritious meals and snacks
  • Foot care services (for an additional fee)
  • A secure environment that feels open, bright and welcoming

What to Expect as a Family Caregiver

Family Caregivers can expect a much-needed break from caregiving while their family members enjoy a safe, friendly and fun environment.

Our collaborative approach to care means that you’ll be in regular communication with our staff team regarding your family member’s welfare and wellbeing. We’re here to answer any questions or concerns you might have throughout your dementia journey.

Being a caregiver can be tough, so don’t forget about yourself. Our Caregiver Education and Caregiver Support Groups can connect you to the services you need to feel supported, capable, and informed.


How to join a Club

To join a Club, you must complete a referral either through the Central Registry or Ontario Health atHome (formerly HCCSS / LHIN) based on your place of residence in Halton. There are waiting lists for our Clubs, so we highly recommend starting the referral process well before your preferred start date.

Referrals for Burlington Residents

To join a Club, Burlington residents must contact Burlington Home and Community Care at 1-800-810-0000 or 905-639-5228

Residents of Oakville, Milton, Halton Hills, Mississauga

To join a Club, you must complete a referral through the Central Registry. Please complete the online request for services form or call 905-281-4443.

The new Dementia Care Centre is named Patty’s Place in honour of local physician, Dr. Patty Plaxton

Patty is living with early onset dementia – a diagnosis that she refuses to let define her. Inspired by her resilience, her friends and family started a grassroots campaign to name the centre after Patty to celebrate her commitment to caring for this community. They raised $1.5M, which helped fund the construction of the new centre.

Dr. Patty Plaxton had a family medical office in Oakville for over 20 years. Outside of work, she has been committed to caring for the community, supporting multiple charity events and foundations. Naming the dementia care centre Patty’s Place solidifies her connection to the wellness of our community – which is in keeping with what she’s been doing her whole life.
