Adult Day Programs

Our Adult Day Programs, known as “Clubs”, are designed for people living with dementia or other challenges and support each person’s individual interests, needs, and abilities.

Each Club features fun activities, enriching programming, great food, and friendly people all in a warm, comfortable atmosphere.  Club Members are encouraged to be as independent as possible, choosing their level of engagement and participation.

Our exceptional team of highly skilled staff spend time getting to know each Club Member so that they can read each individual’s cues. This care and attention to detail means that they can subtly provide the support Club Members need to have a great day and feel at home.

Type of Clubs

Two types of Clubs are available.  Our integrated Clubs (Mississauga and Walmley) are designed for adults who are at risk of social isolation due to a diverse range of challenges, including the early stages of dementia, cognitive impairments and/or physical challenges.  Our dementia-specific programs (Soper and Burlington) are tailored to people living with mid to late stage dementia.

We also offer popular virtual programming that can be accessed at any time from home.

How to Join a Club

To join a Club, you must complete a referral either through the Central Registry or Ontario Health atHome (formerly HCCSS / LHIN) based on your place of residence in Halton.

Residents of Burlington

Residents of Oakville, Milton, Halton Hills, Mississauga

To join a Club, you must complete a referral through the Central Registry. Please complete the online request for services form or call 905-281-4443.

If your relative already has services with Ontario Health atHome, or if you are interested in additional services other than the Adult Day Program, contact Ontario Health atHome.

There are waiting lists for our Clubs, so we highly recommend starting the referral process well before your preferred start date.

If you are having trouble navigating the system, please contact the Club you wish to join and we’d be happy to assist you (see locations below).

It’s also a good idea to tour the Club you wish to join.
